Risk Management Program Coordinator III

The Department of Mental Health's Brockton Multi Service Center (BMSC) is seeking an experienced Director of Risk Management. The Director of Risk management would be responsible for collaborating with the BMSC Director and BMSC Clinical Director in ensuring BMSC's compliance with DMH and Joint Commission regulations, standards and policies. Additional duties and responsibilities of this position include but are not limited to: Reviews all incident reports and ensures they are entered into agency database as well as reviewed at the site's risk committee meeting; oversees the Human Rights complaint process including assignment and review of fact-finding investigations; functions as the Human Rights Coordinator for the center; gathers, reviews and analyzes data regarding risk and related / identified topics, and presents data and trends to Leadership and other designated entities/stakeholders; provides technical assistance regarding incident reporting, risk database processes and related policies as needed. The Director of Risk Management will serve as the lead for Performance Improvement for BMSC via coordinating, monitoring, developing, implementing and reporting on PI projects. They will also serve in conjunction with the BMSC Clinical Director as the liaison to the Joint Commission (TJC), Centers for Medicare and Medicaid (CMS), and other regulatory agencies by coordinating, participating and overseeing all TJC and regulatory activities for the Center. This position is 40 hours per week, Work Schedule: 8:30 a.m. to 5:00 p.m. The days off are Saturday and Sunday.

Coordinates center-wide risk management activities with all programs to ensure compliance with established standards. Reviews all incident reports and assures proper follow up and reporting is accomplished within established timelines. Oversees the critical incident reporting process including ensuring that all critical incidents are reported and that any necessary follow up is completed. Chairs the BMSC Risk Management Committee by facilitating regularly cadenced meetings, ensuring minutes are kept, supporting follow up occurs regarding committee requests by obtaining necessary information, and critical information is shared with necessary parties. Ensures that clients identified to be on the Area Risk Review list are reviewed at least monthly at the BMSC Risk Committee and that up-to-date information is presented quarterly to Area office. Oversees the Human Rights complaint process including assignment of fact-finding investigations. Provides critical review of DMH investigations and other fact-findings as well as processes and tracks decision letters, corrective actions and appeals in the Human Rights Complaint process. Writes draft decision letters as designee on behalf of the Center Director who reviews for distribution. Oversees the CPPD, Elder Abuse, Child Abuse and other mandated reporting processes for the center to ensure that reporting occurs as established per mandatory reporting guidelines. Serves as liaison to Office of Investigations and Area office regarding risk management activities. Provides consultation and training regarding identification and management of risk issues as well as how to use the incident reporting database to site staff in order to enhance staff risk management knowledge. Uses Tableau and other identified databases to gather and analyze data as required. Provides statistical risk management information to BMSC and Area Leadership as requested for all programs. Participates in various committees such as Leadership, Risk, EOC, Policy and other committees as necessary related to Risk Management, Joint Commission and Performance Improvement activities. May review client records to ensure the Center's compliance with CMS regulations and the Joint Commission standards. Functions as a lead Joint Commission Coordinator to assist with and ensure compliance with Joint Commission standards and CMS regulations; oversees the Center's preparations for all regulatory surveys as well as any needed follow-up. Serves as the primary liaison for the Center with representative contacts at the regulatory agencies. Oversees all Performance Improvement Teams (PITs) by providing leadership, guidance and staff training in the PI process. Participates in State and Area wide Joint Commission standards groups and participates in conducting mock surveys across the Area.

Preferred Qualifications: Proficiency in Microsoft Office applications including Outlook, Word, and Excel. Specialized training and/or certification in areas related to performance improvement, quality assurance and compliance, data analysis and reporting.

The Brockton Multi Service Center provides comprehensive services to mentally ill adults and children. Programs include Case Management, Adult Community Clinical Services program and a Program of Assertive Community Treatment (PACT) team. The Brockton Multi Service Center is a Joint Commission certified facility.
