Our Surgery Chart Loggers play a key role in ensuring that surgical documentation is complete, accurate, and ready for billing prior to submission. The Surgery Chart Logger serves as an essential checkpoint between clinical documentation and the billing process, helping prevent delays, denials, and lost revenue.

As a member of the Revenue Integrity team, you'll review surgical charts, verify required documentation and charges, reconcile case details, and work closely with surgeons, anesthesia providers, nursing staff, and coding and billing teams to ensure each case is complete and ready for billing.

Surgery Chart Logger Details

Essential Job Duties:

• Performs comprehensive pre-billing chart audits for surgical cases.
• Ensures all required clinical documentation is complete and available, including:
  • Operative reports
  • Anesthesia records
  • Nursing documentation
  • Implant and supply documentation
• Verifies that procedures performed are properly documented and ready for coding and billing.
• Confirms all applicable charges are present and supported by clinical documentation.
• Identifies and resolves:
  • Missing documentation
  • Incomplete records
  • Unlinked or unsupported charges
• Works charts until they are fully complete before releasing cases to billing.
• Maintains timely chart reconciliation and release to billing to minimize charge delays.
• Tracks and manages work queues across multiple operating room locations.
• Prioritizes high-volume and high-revenue cases to ensure timely processing.
• Communicates directly with surgeons, anesthesia providers, and nursing staff regarding missing or incomplete documentation.
• Follows up on outstanding documentation to ensure surgical charts are complete.
• Partners with coding and billing teams to ensure a smooth handoff.
• Demonstrates accuracy, attention to detail, dependability, and effective communication.
• Maintains confidentiality and follows applicable hospital policies and procedures.
• Works collaboratively with clinical and revenue integrity teams to support accurate and timely billing.
• Seeks personal and professional growth while completing assigned responsibilities.

Compliance & Accuracy

• Ensures charts meet organizational, regulatory, and payer documentation requirements.
• Supports audit readiness by maintaining accurate and complete records.
• Identifies and escalates recurring documentation issues or trends.
• Ensures documentation supports charges and procedures before cases are released for billing.
• Maintains compliance with established documentation, privacy, and revenue integrity standards.

Qualifications

Education

• High school diploma required; associate or bachelor’s degree preferred.

Experience

• 3–5+ years of experience in:
  • Operating room or perioperative services
  • Medical records, chart review, or revenue cycle
• Strong preference for surgical services experience.

Skills

• Strong attention to detail and accuracy.
• Knowledge of:
  • Surgical workflows
  • Basic CPT/charge capture concepts
  • Clinical documentation requirements
• Ability to manage high-volume workloads.
• Ability to identify missing, incomplete, or unsupported documentation.
• Strong written and verbal communication skills.
• Ability to collaborate effectively with clinical, coding, billing, and revenue integrity teams.
• Experience with EMR systems, Epic preferred.
• Ability to prioritize work and meet established deadlines.
• Ability to maintain confidentiality when handling sensitive clinical information.
