Job description

Care Coordinator

The Care Coordinator works in partnership with RN Case Manager and with the health care team to coordinate patient care and discharge/transitional support by providing administrative and clinical support to patients and families. They assist with referrals to organizational and community resources as identified by the team. May participate in the rounding process as indicated, based on schedule set by providers. They schedule/coordinate follow up appointments, coordinate scheduling of follow up studies and procedures, and facilitate communication with Primary Care Providers (PCPs) including assisting with scheduling PCP appointments. Care Coordinators closely collaborate with RNCMs, team members and providers to coordinate/manage referrals to agencies for services, equipment and supplies. Independent activities related to coordination of care include communicating clinical information to internal and external customers.

Required Education and Experience High school diploma or equivalent experience. Minimum of four (4) years experience with progressive responsibilities in health care facility administrative support functions such as unit coordinator, insurance processing coordinator, patient care coordinator, MA, or related roles. Required Credentials N/A. Preferred Bachelor's degree in a health related field. Previous experience in a acute health care setting. Previous experience coordinating care for complex pediatric populations.

Compensation Range

$29.16 - $43.73 per hour

Salary Information

This compensation range was calculated based on full-time employment (2080 hours worked per calendar year). Offers are determined by multiple factors including equity, skills, experience, and expertise, and may vary within the range provided.
