Health And Wellness Coordinator & Case Manager

COPAL, Communities Organizing Latine Power and Action, is a member-based organization established in 2018 to improve the quality of life of Latine families. $25.00 $26.00 per hour depending on qualifications and level of experience

This is a regular, full-time position that will be fully in person. It requires some availability to work on evenings and weekends as needed for community/program events and requires the ability to travel to locations throughout the Twin Cities metro area and other sites as required by program activities.

COPAL is hiring a Health and Wellness Coordinator & Case Manager to plan, coordinate, and execute health and wellness initiatives. They will manage program activities, establish partnerships with community organizations, support community outreach and advocacy efforts, and actively engage community members to support them in accessing resources and navigating health systems.

Plan and Coordinate Program Activities

  • Collaborate with the Health and Wellness (H&W) team to fulfill contract requirements and achieve program goals.
  • Coordinate key program activities, including health fairs, vaccination events, educational workshops, mental health programs, community discussions, community health campaigns, and other initiatives.
  • Manage project timelines, allocate resources, and oversee logistics to ensure programs are delivered on schedule and within budget.
  • Collaborate with the H&W team in designing outreach strategies to engage target populations and improve program participation.
  • Support coordination across COPAL departments when H&W initiatives intersect with other departments and organizational priorities.
  • Support in identifying implementation challenges, emerging community needs, and opportunities for program improvement.

Case Management and Resource Navigation

  • Complete and maintain intake records and required documentation accurately and confidentially.
  • Assess the needs, goals, and strengths of individuals and connect them with the appropriate community partners, agencies, programs, and other resources based on their needs.
  • Provide hands-on assistance with applications and documentation for services and benefits, as requested.
  • Offer follow-up support to promote successful connections, continuity of care, and meaningful outcomes.
  • Deliver culturally responsive, trauma-informed, and strengths-based support that honors lived experience and self-determination.
  • Work alongside COPAL's navigators and program staff to triage needs, support service navigation, and coordinate appropriate solutions.
  • Identify recurring gaps or systemic barriers affecting community members and communicate trends to the H&W Director for programmatic or advocacy follow-up.

Community Engagement, Partnerships, and Advocacy

  • Build and sustain strong, collaborative relationships with local service providers, community-based organizations, public agencies, local businesses, health systems, coalitions, and other stakeholders relevant to the department.
  • Coordinate and facilitate presentations that share information about COPAL's programs, services, community impact, health priorities, and available resources.
  • Represent the organization at networking and outreach events, community meetings, coalitions, working groups, public forums, and collaborative initiatives.
  • Develop and implement strategies to engage community members, increase program awareness, and drive participation.
  • Organize and participate in community events, such as health fairs and information sessions to promote access to health resources and advance community health priorities.
  • Gather community perspectives, stories, needs, and concerns through outreach, conversations, listening sessions, and program activities and ensure these perspectives inform H&W programming and advocacy.
  • Support advocacy initiatives related to health access, health equity, social determinants of health, and other issues affecting COPAL communities.
  • Help translate community experiences and program data into information that can support policy recommendations, educational materials, advocacy messages, and organizational decision-making.
  • Collaborate with COPAL's organizing and communications teams to ensure H&W advocacy efforts are coordinated with broader organizational strategies and campaigns.

Data, Documentation, and Program Learning

  • Contribute to data collection and reporting that supports collaboration, planning, continuous improvement, equitable resource allocation, funder reporting, and assessment of program impact.
  • Document services provided, referrals, outcomes, and exit information accurately and consistently.
  • Maintain organized, timely, and confidential case notes and data records.
  • Track participation, referrals, service outcomes, outreach activities, and other relevant program indicators.
  • Help identify trends emerging from case management and community engagement data, including recurring barriers to services or health access.
  • Participate in periodic review of program results and use findings to recommend adjustments to outreach, service delivery, partnerships, and advocacy strategies.

What You Bring

  • A bachelor's degree in public health, social work, psychology, counseling, or a related field, or 1-3 years of equivalent experience.
  • You are bilingual in Spanish and English with the ability to communicate effectively with community members verbally and in writing.
  • Previous experience working with diverse populations.
  • Previous experience with group facilitation and community presentations.
  • Previous experience in social services, community outreach, case management, or nonprofit work is preferred.
  • Previous experience coordinating programs, events, outreach initiatives, or community-based projects is preferred.
  • Previous experience building relationships with community organizations, public agencies, healthcare providers, or other external partners is preferred.
  • Knowledge of Twin Cities and Minnesota community resources is preferred.
  • An understanding of, or commitment to learning about, progressive theories of race, gender, class, and intersectionality, with the ability to apply these frameworks to support equitable and inclusive engagement across diverse communities.
  • The ability to complete forms accurately and manage case files and program documentation with strong attention to detail.
  • The ability to establish relationships and assist in individual and group conflict resolution.
  • The ability to work with diverse communities in a respectful, culturally responsive, and competent manner.
  • The ability to maintain confidentiality and protect sensitive information.
  • The ability to support advocacy efforts while centering the experiences, priorities, and voices of community members.
  • Strong verbal communication, presentation, and facilitation skills in both Spanish and English.
  • The ability to work independently, establish priorities, and meet deadlines.
  • Strong organizational skills with the ability to manage multiple priorities.
  • An understanding of responsive community and customer service principles and conduct.
  • Strong computer skills, including proficiency in Microsoft Office and the ability to learn databases, CRM systems, and project management tools.
  • The ability to work flexible hours, including evenings and weekends, as required by program or community activities.
  • The ability to travel to locations throughout the Twin Cities metro area and other sites as required by program activities
