Case Worker
Location: New York, NY
Contract: 13 Weeks
Shift: Day | Monday–Friday
Shift Time: 9:00 AM–5:00 PM
Hours: 40 Hours/Week
Pay Rate: $1,500–$1,720/Week
Responsibilities
Provide social services and care coordination to patients, families, and individuals requiring assistance.
Assess client needs and develop appropriate social service plans.
Determine eligibility for applicable social and community-based services.
Conduct face-to-face and telephone care coordination.
Assist patients and families with healthcare and social service system navigation.
Facilitate referrals for medical care, mental health, substance-use treatment, housing, transportation, home care, DME, hemodialysis, respite care, and other services.
Conduct post-discharge follow-up as required.
Provide support with facility placement activities.
Distribute and coordinate discharge notices.
Assist the Primary Social Worker with assigned tasks, including Next of Kin (NOK) searches.
Provide counseling and case-related support.
Maintain accurate and timely documentation in Epic and other required systems.
Coordinate schedules effectively for patient engagement, field activities, follow-up, and administrative responsibilities.
Maintain effective communication with patients, families, healthcare providers, and community organizations.
Requirements
Bachelor's/Baccalaureate Degree from an accredited college or university.
3 years of relevant experience required.
Epic experience required.
Facility placement support experience.
Referral facilitation experience.
Next of Kin (NOK) search experience.
System navigation and care coordination experience.
Post-discharge follow-up experience.
Strong communication, organizational, and documentation skills.
Ability to work independently in patient-facing, field, telephonic, and administrative settings.
English proficiency required in reading, writing, and speaking.
