Compensation: $102,000 - $140,000 depending on verification of experience and qualifications.

Position Purpose: The Quality of Care Coordinator is responsible for managing the daily clinical operations of the facility to ensure that high quality nursing care is delivered to residents in a resident-centered manner. Coordinates the activities to ensure excellence in quality outcomes/indicators and leads the Quality Assurance/Performance Improvement program. Ensures regulatory compliance in all clinical matters. Leads the clinical team in the creation and successful management of a budget. Leads the infection control program.

What You Will Do:

  • Performs daily rounds to assess the resident and to evaluate the effectiveness of the clinical care plan.
  • Works in the capacity of a licensed nurse for one shift per month to evaluate clinical care delivery systems.
  • Assures the effectiveness of the infection control program.
  • Responsible for the interviewing, hiring, orientation, development, corrective action and evaluation of staff to ensure quality care to residents. Completes annual and other required performance appraisals and competencies on all staff members in a timely and effective manner.
  • Reviews and assures the staff compliance with in-service requirements and program for self-development; Assures a program of staff development that supports growth of staff skills.
  • Leads the care planning process and assures that holistic resident centered care is provided.
  • Monitors QI's and coordinates a quality improvement program with the individual house Clinical Support Team; Plans, implements, and maintains records for the quality assurance/C.Q.I. program.
  • Manages admissions; Creates and reviews key reports monthly from the Electronic Medical Record.
  • Creates new clinical policies and procedures in conjunction with the clinical team members.
  • Reviews all emergency management protocols and conducts drills, reviews and education as needed.
  • Participates in all meetings as recommended by regulatory agencies such as DPH, OSHA, and Joint Commission.
  • Responsible for the MMQ program as required by State Regulations; Assures compliance with MDS regulation and monitors collection and submission of MDS data.
  • Maintain department budget, place orders for equipment and supplies as necessary. Identify financial vulnerabilities and make cost reduction recommendations as needed.
  • On-call requirement.

Minimum Qualifications:

  • BSN and Valid RN license in the state of Massachusetts required.
  • Three (3) years of Supervisory experience preferred; Long-term care experience preferred.
  • Knowledge of Joint Commission and DPH standards preferred.
  • Demonstrates superior written and verbal communication and presentation skills appropriate for audience comprehension.
  • Proven ability to develop and implement budget.
  • Must be able to see, hear and smell or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully.
  • Must be able to speak, read and write the English language in an understandable manner.
  • Primarily works inside with frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 25 pounds to a minimum height of 3 feet and be able to push, pull, move and or carry such weight a minimum distance of 10 feet. May be necessary to assist in the evacuation of residents in emergency situations.
