Program Care Coordinator, Transitional Care Program

Status: Full-time | Location: Remote/ Hybrid at Tempe Clinic

Schedule: Monday through Friday 8:30 am - 5:00 pm

Position Summary

The Program Care Coordinator provides administrative, coordination, and operational support across CPR programs to ensure timely access to care, continuity of services, and effective communication between internal teams and external partners. This role supports referral processing, scheduling, documentation, outreach, and coordination of care while upholding CPR's standards for professionalism, accuracy, and client-centered service.

Program Care Coordinators work collaboratively with clinical teams, leadership, and community partners to support program operations and ensure contractual, clinical, and service timelines are met.

Knowledge and Skills

  • Communication Skills: Ability to communicate clearly, calmly, and professionally with clients, staff, providers, and external partners, including in sensitive or high-pressure situations.

  • Organizational & Multi-Tasking Skills: Ability to manage competing priorities, track tasks accurately, and meet deadlines in a fast-paced environment.

  • Behavioral Health Awareness: Comfort interacting with individuals experiencing acute or chronic mental health needs, with an emphasis on professionalism and empathy.

  • Computer & Phone Proficiency: Ability to document accurately in electronic medical record systems and communicate effectively via phone and email.

  • Demonstrating strong typing skills for efficient/accurate documentation

  • Attention to Detail: Strong accuracy in documentation, data entry, scheduling, and follow-up.

  • Teamwork & Independence: Ability to work independently while contributing effectively to a collaborative team environment.

  • Customer Service Orientation: Commitment to upholding CPR's core values and providing respectful, responsive service.

Primary Duties and Responsibilities

  • Respond to phone calls, emails, and referrals in a timely and professional manner.

  • Coordinate referrals, scheduling, and documentation in CPR's electronic medical record and tracking systems.

  • Communicate with internal teams, providers, and external partners to support continuity of care.

  • Conduct outreach to clients, referral sources, and community partners as required.

  • Accurately collect, enter, and maintain client, referral, and service data.

  • Monitor timelines and follow up to ensure program and contractual requirements are met.

  • Identify and escalate concerns or barriers to supervisors as appropriate.

  • Maintain confidentiality and comply with all HIPAA, CPR, and departmental policies.

  • Provide cross-coverage and support within the department as needed.

  • Reliable and predictable attendance is an essential job function.

  • Complete other duties as assigned.

Transitional Care Program (TCP)

Additional Responsibilities Include:

  • Gather and document client demographics, insurance, and referral information.

  • Verify insurance coverage, billing codes, and referral requirements.

  • Create charts, complete screenings, and support intake scheduling.

  • Monitor caseloads for follow-up needs and lack of contact.

  • Coordinate care with providers, billing teams, insurers, and community partners.

  • Support providers with outreach and documentation.

  • Track safety check-ins related to new intakes during business hours.

  • Assist with patient resources, referrals, and skills-based support as appropriate.

Qualifications and Education Requirements

  • High School Diploma or GED required.

  • Minimum of 2 years of experience in behavioral health, healthcare administration, care coordination, or a related field.

  • Associate's or Bachelor's degree in Social Work, Behavioral Health, or a related field preferred.

  • Ability to qualify as a Behavioral Health Technician (BHT), if required by program.

  • Relevant experience may be substituted for education where appropriate.
