Job description

Statewide Subject Matter Expert for PITP Program

Serves as a statewide subject matter expert for HCPF's Permanently Incompetent to Proceed (PITP) program, providing expertise and guidance on program design, policy implementation, care coordination, crisis system navigation, and cross-system placement and stabilization. This position works collaboratively across HCPF and with external partners, including Eligibility Sites, Case Management Agencies (CMAs), CDHS, CDPHE, OPG, RAEs, healthcare and community partners, and the criminal justice system, to coordinate complex cases, address systemic barriers, and support safe and appropriate member placements. The position develops and operationalizes statewide PITP care coordination standards and cross-system placement pathways to promote continuity of care, effective resource utilization, legal and program compliance, and community safety.

Key Responsibilities

  • Lead intensive coordination and stabilization planning for PITP members with intellectual and developmental disabilities (I/DD), neurocognitive disorders, complex support needs, and significant justice-system involvement.
  • Review Medicaid eligibility information and coordinate the collection of documentation needed to support eligibility and access to services.
  • Partner with Case Management Agencies (CMAs), Regional Accountable Entities (RAEs), providers, counties, crisis teams, families, and state and criminal justice partners to develop stabilization and placement solutions.
  • Develop and implement processes for secure and step-down placement planning while considering member needs, public safety, facility security requirements, judicial conditions, and continuity of care.
  • Identify and resolve barriers involving Medicaid benefits and eligibility, waiver access, provider capacity, crisis protocols, facility requirements, and placement availability.
  • Identify systemic barriers affecting PITP members and develop statewide operational solutions and standards to support consistent service delivery.
  • Determine when complex cases require cross-agency escalation, additional subject matter expertise, or modifications to stabilization and placement plans.
  • Provide Department leadership with program updates, recommendations, risk-mitigation strategies, and lessons learned to inform program decisions and legislative recommendations.
  • Serve as a primary HCPF liaison for PITP and complex-needs coordination across state agencies, healthcare and community systems, and criminal justice partners.
  • Coordinate with CDHS, CDPHE, RAEs, OPG, CMAs, courts, probation, public defenders, sheriffs, community corrections, crisis services, providers, and internal HCPF programs.
  • Facilitate cross-system planning related to secure and step-down placements, shared care plans, safety planning, crisis intervention, stabilization, and transitions between levels of care.
  • Facilitate resolution of complex issues involving multiple agencies, service systems, regulatory requirements, and funding responsibilities.
  • Coordinate with HCPF legal, financial, eligibility, policy, and operational teams to evaluate Department-wide impacts of potential program, regulatory, or waiver changes.
  • Manage PITP-related interagency agreements and participate in recurring statewide coordination activities.
  • Develop and refine statewide PITP operational guidance, workflows, procedures, stabilization and placement pathways, and interagency coordination protocols.
  • Draft policy recommendations, operational guidance, implementation materials, and resources supporting PITP and complex-needs populations.
  • Develop and monitor measures related to member outcomes, secure placement utilization, program performance, and systemic barriers.
  • Identify and prioritize policy and program improvements affecting access to LTSS, stabilization services, and continuity of care.
  • Interpret and apply applicable judicial orders, statutes, regulations, waiver requirements, and HCPF policies to support compliant program operations.
  • Engage state agencies, CMAs, RAEs, criminal justice partners, providers, and other stakeholders to identify system needs, barriers, and opportunities for improvement.
  • Research national practices and emerging approaches to inform PITP program design and improve member outcomes.
  • Recommend statewide policy and program improvements to Department leadership based on data, stakeholder feedback, operational experience, and identified system gaps.
  • Provide technical assistance to CMAs, RAEs, counties, providers, and criminal justice partners on PITP requirements, care coordination, secure placement, safety planning, and cross-system navigation.
  • Develop and deliver training on PITP program requirements, disability-responsive care coordination, secure placement processes, and complex system navigation.
  • Develop guidance documents, tools, resources, and communication materials that support consistent statewide implementation.
  • Identify compliance concerns involving statutes, waivers, regulations, policies, or established processes and provide guidance to support appropriate and consistent implementation.
  • Identify inconsistencies or gaps among statutes, regulations, waiver requirements, and HCPF policies and recommend clarification or revision.
  • Use program data, case experience, and stakeholder feedback to identify training, technical assistance, and system-improvement needs.
  • Consult with HCPF Legal or the Attorney General's Office when legal interpretation or guidance is necessary.
