Licensed Vocational Nurse (LVN)

Job Description:

Education/ Certification Mandatory:

• Valid/current Texas LVN license

Work Experience Mandatory:

• Minimum 2 years of LVN experience; experience providing nursing care, medication administration, treatments, documentation, and patient/resident care

Education/Certifications Nice to Have:

• Valid/current Texas LVN license

Work Experience Nice to Have:

• Experience with intellectual/developmental disabilities (IDD), complex medical needs, mental health and/or behavioral health populations; experience in a State Supported Living Center/state facility would be highly desirable

References/Certificates/Diplomas Required:

• Two complete references from two different places of employment by supervisor of the nurse
• A NURSYS report showing any licenses held (must be current)
• Shift preference (6/2 or 2/10)

Roles And Responsibilities:

• Provide direct nursing care to residents with intellectual disabilities, complex medical conditions, mental health and/or behavioral health needs.
• Administer medications and treatments in accordance with physician orders and facility procedures.
• Monitor residents' health status, vital signs and changes in condition, and report concerns appropriately.
• Provide medication education and reinforce care instructions with residents and/or appropriate staff.
• Complete accurate and timely clinical documentation and computer data entry.
• Follow facility SOPs, ICF-IDD Regulations, DSSLC Policies and Directives, and HHSC Standards, Rules and Directives.
• Support resident safety, general welfare and quality of care within the State Supported Living Center environment.
• Participate in required facility and HHSC training and maintain compliance with all applicable nursing and facility requirements.
