Transitional and Bridge Care Coordinator RN
Signature Eugene Hospice is seeking a compassionate, relationship-driven Transitional and Bridge Care Coordinator RN to support seamless patient transitions from Home Health into hospice care. This role collaborates with our home health to coordinate referrals, facilitate admissions, and ensure patients receive timely, compassionate care.

Key responsibilities include coordinating hospice referrals and admissions from hospitals, SNFs, physician offices, and other healthcare partners, educating referral sources on hospice services and patient eligibility, collaborating with clinical and administrative teams to ensure exceptional patient care and customer service, participating in community outreach, networking, and business development activities, maintaining accurate documentation and timely follow-up on referrals, and supporting agency growth by identifying opportunities to serve patients and healthcare partners.
Qualifications include current Oregon RN license, graduate of an accredited nursing program, 1-2+ years of nursing experience required, hospice, home health, case management, discharge planning, acute care, or transitional care experience preferred, excellent communication, relationship-building, and organizational skills, valid driver's license, and reliable transportation.
