  • Help patients understand medications, blood glucose monitoring, lifestyle changes, and self-management strategies.
  • Follow patients over time and adjust education plans based on their goals and progress.
  • Coordinate and support the day-to-day operations of the diabetes education program.
  • Partner with department leadership to plan program priorities, goals, and improvement initiatives.
  • Ensure program compliance with diabetes education standards and accreditation requirements.
  • Review educational materials and update content to meet the needs of diverse patient populations.
  • Work alongside providers and interdisciplinary teams to improve care coordination and patient outcomes.
  • Develop and deliver educational programs for caregivers, providers, and staff.
  • Participate in quality improvement projects that strengthen diabetes care across the organization.
  • Serve as a clinical resource for diabetes education and best practices.

Minimum Qualifications

Education

  • Bachelor's degree in a related field preferred.

Experience

  • At least one year of diabetes education experience or an equivalent combination of education and experience.

  • Outpatient clinical experience preferred.

Credentials

  • Basic Life Support (BLS) required upon hire.

What Will Help You Stand Out

Experience is important, but we're also looking for someone who enjoys teaching, building relationships, and helping patients feel empowered in their healthcare journey.

Candidates may be especially successful if they have:

  • Experience in outpatient endocrinology, diabetes management, nutrition counseling, or chronic disease education.
  • Previous involvement with program development, accreditation, or quality improvement initiatives.
  • Strong presentation and education skills across diverse patient populations and learning styles.

A Few Things We'd Love to See

If you've developed educational content, led patient education initiatives, or helped improve clinical programs, we'd love to hear about it. We recognize that great candidates bring different experiences to the table, and we encourage you to apply even if your background doesn't match every preferred qualification.
