Angelicare Home Health Inc is looking for a passionate and dedicated Home Health Physical Therapist to join our growing team in Temecula, CA. Responsibilities:
- Conduct comprehensive in-home physical therapy evaluations and develop individualized treatment plans
- Provide skilled PT interventions to improve patient mobility, strength, balance, and functional independence
- Monitor and document patient progress, adjusting care plans as clinically appropriate
- Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
- Collaborate with the interdisciplinary care team including nurses, OTs, and physicians to ensure coordinated care
- Maintain accurate and timely clinical documentation in compliance with Medicare and state regulations
- Participate in care conferences and contribute to discharge planning
Requirements:
- Active California Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Home health or outpatient PT experience preferred
- Knowledge of Medicare home health guidelines and OASIS documentation
- Valid driver's license, reliable transportation, and current auto insurance
- Strong clinical assessment, critical thinking, and patient communication skills
- Ability to work independently and manage a flexible caseload in the field
