Key Responsibilities
- Provide speech-language pathology services to patients within a home health setting across an assigned territory
- Conduct evaluations and deliver therapeutic interventions addressing communication, swallowing, and related disorders
- Maintain a productivity standard of 30 points per week (approximately 6 points per day)
- Document patient care accurately and in a timely manner using HCHB (Homecare Homebase) software
- Collaborate with interdisciplinary team members to support patient goals and care plans
Required Qualifications
- Active Speech-Language Pathologist (SLP) license in the state of Oregon
- Previous home health experience required
- Previous experience with HCHB (Homecare Homebase) required
- Ability to travel within a 50-mile radius of the office
Preferred Experience / Skills
- Experience managing a caseload with productivity expectations in a home health environment
- Strong organizational and time-management skills to support an independent, field-based schedule
