As our RN Case Manager, you'll orchestrate meaningful care that touches hearts and creates lasting comfort for families during their most vulnerable moments. This role goes beyond medical tasks – you'll be the steady, compassionate presence that makes all the difference.

· Lead with Heart: Guide a multidisciplinary team of LPNs, Aides, Social Workers, and Chaplains in creating personalized care experiences

Your Impact Zone: As an RN Case Manager, you'll travel throughout the Dayton area, bringing healing and hope directly to patients in their homes, assisted living facilities, long-term care facilities, and hospitals –wherever they feel most comfortable.
What You Bring & What We Offer
Required Qualifications:
Active RN License
Valid Driver's License
1+ years hospice experience
Preferred Qualifications:
BLS Certification
Hospice and Palliative Medicine background
