This contract role as a Behavioral Health Care Coordinator in Chicago, IL is responsible for providing care coordination, which includes conducting health assessments, creating care plans, educating members on health care programs to address their personal health plan needs, engaging the member in discussion of adherence to personal health goals, and completing in-person visits to members living in long-term care facilities.
Pay Rate: $40.50 - $43.50 per hour
Job Requirements:
•Licensed Professional Counselor (LPC), Licensed Clinical Professional Counselor (LCPC), Licensed Master Social Worker (LMSW/LSW), or Licensed Clinical Social Worker (LCSW) with 2 years direct clinical care to the consumer in a clinical setting.
•Current, valid, unrestricted license in the state of operations (or reciprocity).
•Plus 4 years wellness or managed care experience presenting clinical issues with members/physicians.
•Experience handling medical management programs and health educations programs in an independent manner.
•Knowledge of the health and wellness marketplace and employer trends.
•Perform Transition of Care services and support members post-discharge
•Verbal and written communication skills including discussing medical needs with members and interfacing with internal staff/management and external vendors and community resources.
•Analytical experience including medical data analysis.
•PC proficiency to include Word, Excel, and PowerPoint, database experience and Web based applications.
•Current state driver license, transportation, and applicable insurance
•Ability and willingness to travel
