About the Position

As directed by the Director of Nursing Services, assists with the monitoring and coordination of the delivery of health care services for people with autism who are living in community based homes.

Responsibilities

  • Monitor, coordinate and provide delivery of preventative and routine health care for participants with autism who reside in agency’s community based homes.
  • Maintain medical records at the Residential site including updating medical orders, and other pertinent medical records.
  • Review monthly MAR’s and complete weekly cabinet/ MAR checks.
  • Attend annual Life Plan meetings.
  • Adhere to and assist with ensuring facility compliance with all physicians, NP, PA, etc. medical-related orders.
  • Assist with coordinating medical program needs with community services, medical providers and other related agencies.
  • Monitor/oversee to ensure adequate and timely scheduling and completion of health related exams, appointments, laboratory/diagnostic testing as ordered/required; ensure that all follow up recommendations are implemented.
  • Accompany participants to medical appointments if necessary.
  • Safely transports participants to medical appointment if necessary.
  • Assist with the monitoring of medication administration in the group homes; offers support, guidance and recommendations to ensure medication administration is done properly.
  • Administer medications, treatments and supplements as necessary/per MD orders.
  • Assist with providing health services training for staff.
  • Participate as an active member of the Interdisciplinary Team.
  • Attend monthly residential team meetings as assigned.
  • Maintain compliance with and assist with ensuring facility compliance with all applicable State/Federal regulations and agency policies and procedures.
  • Maintain communication with residence manager, care coordinator and all members of the Interdisciplinary Team as necessary.
  • Assist with teaching monthly First Aid training for new staff.
  • Assist with conducting nursing chart/ medical audits.
  • Performs all other duties as assigned.
  • Responsible for being on time to every scheduled work shift and recording hours worked accurately via Paylocity or timeclock punches.
  • Responsible for ensuring that confidentiality is strictly maintained as required by law and Agency policy.
  • Responsible for securing and protecting personal and agency related data by adhering to the Agency’s computer security policies
  • Maintain all agency trainings and certifications.
  • Physical agility and ability to react to emergency situations, including Strategies for Crisis Intervention and Prevention – Revised (SCIP-R) and CPR certification as per regulatory guidelines.
  • Follow all agency policies and procedures.

Qualifications

  • Current New York State license to practice as a Licensed Practical Nurse.
  • Valid New York State driver’s license.
  • Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, grasping, bending, stooping, squatting, kneeling and climbing.
  • Ability to move, reposition, and lift and transfer individuals as required by prescribed program/ plan.
