Job Summary

Clinical Expectations: Under general supervision, works as full member of the MICU multi-professional critical care team. Contributes to excellence in patient care, research, teaching, and provides leadership to the organization. Functions as a medical intensive under the supervision of the MICU Director or designee. Collaborates with resident physicians in internal medicine and manages the critical care aspects of the assigned patient's intensive care stay in the MICU and progressive care unit (PCU) from admission to transfer or discharge. Demonstrates an advanced level of medical and nursing knowledge, clinical/technical competence including expertise in central line insertion, airway intubation, arterial cannulation, lumbar puncture and similar procedures.

The Critical Care Nurse Practitioner will additionally have responsibility for quality and performance metrics and improvements in conjunction with the MICU leadership and will be required to contribute to educational and research activities in the MICU/PCU.

Administrative Expectations: Oversee professional, clinical, and educational activities of the department's APPs. Ensure a culture of open dialogue and 360o feedback with all staff, promote healthy team norms, and develop pathways to support staff and promote wellness. Promote a culture of diversity and inclusion within the Department. Proactively identify and address areas of inequity. Recognize and promote the value of enhancing diversity in the healthcare workforce. Promote and support a system of recruitment, development, and succession planning for APPs and other staff leadership positions, with a focus on developing opportunities for professional advancement for APPs. Promote implementation of and adherence to standardized clinical workflows. Establish open non-judgmental dialogue, welcome constructive criticism, and demonstrate timely responsiveness. Maintain appropriate levels of transparency and respectful accountability. Effectively manage disagreements among staff.

The Lead APP will serve as a liaison between the department's APPs, department's division heads, and working closely with unit nurse manager. The Lead APP will be in charge of scheduling for the group


Essential Functions :

  • Performs and documents in the medical record a complete history and physical examination for acute and complex chronically ill patients. (15%)
  • Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Formulates differential diagnoses by priority. Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address immediate critical care needs. Incorporates clinical research findings in the development and implementation of standards of care. Utilizes current research and evidence-based decision-making in all clinical practice. (15%)
  • Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. (10%)
  • Coordinates daily rounds with the Attending and Intensivist physician teams, presents 24 hour patient report, and collaborates in plan of care. Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions. (10%)
  • May serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions. (10%)
  • Prescribes diagnostic strategies and therapeutic interventions both pharmacologic, non-pharmacologic and surgical, needed to achieve expected outcomes. Prescriptive authority for prescribing, ordering, and or administering drugs; including (but not limited to) schedule II, III, IV, and V drugs and necessary medical devices. (5%)
  • May perform advanced procedures consistent with privileges and competency validation, including minor suturing, complex wound management, line and tube placements and removals. (5%)
  • Provides supervisory leadership for the MICU NP program including scheduling, recruitment and retention, credential review and continuing evaluation and assesment. (10%)
  • Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and/or clinic note where applicable. (H&P, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries in medical record per specific patient unit or service, death summary) (5%)
  • Assesses educational needs of the patient-care staff and provide one-on-one as well as formal education as needed. Maintains ongoing communication and collaboration with interdisciplinary health care team. Implements and evaluates appropriate educational programs for DHMC MICU patient care staff and other healthcare providers as appropriate. (5%)
  • Assesses educational needs of the patients / families and provide culturally-sensitive patient/family education interventions as indicated. Use non-technical language when educating patients and families. Document all pre and post-care education in patients' medical record. (5%)
  • Collaborates with MICU, SICU and PCU leadership in advancing cross-unit, cross-disciplinary performance initiatives. Actively participates in Grand Rounds, APN council meetings, faculty and unit meetings, M&M presentations. (5%)


Education :

  • Master's Degree From an accredited program as a nurse practitioner Required


Work Experience :

  • 1-3 years Critical Care NP Required
  • 1-3 years Critical Care nursing Required


Licenses :

  • ACLS-Advanced Cardiac Life Support - AHA - American Heart Association Required and
  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association Required and
  • NPRXN-Nurse Practitioner Prescriber - DORA - Department of Regulatory Agencies Required and
  • NP-Nurse Practitioner - AANPCB - American Academy of Nurse Practitioners Certification Board Required or
  • NP-Nurse Practitioner - DORA - Department of Regulatory Agencies Required


Knowledge, Skills and Abilities :

  • Knowledge of medical principals and procedures.
  • Knowledge of community resources for appropriate procedures.
  • Skill in eliciting history from patients, performing physical exams and formulating assessments of and treatment plans for patients.
  • Ordering appropriate diagnostic studies to aid in diagnosis.
  • Knowledge of appropriate administration and prescription of medications.
  • Skill in utilizing the principles and practices of effective and persuasive communication to elicit information, negotiate problem resolution, and/or garner support for various programs or policies.
  • Skill in obtaining information from patients, calming and reassuring patients and motivating patients to accept treatments. Ability to exercise initiative and judgment in selecting proper treatment.
  • Ability to respond appropriately in emergency situations.
  • Ability to prepare accurate reports.
  • Ability to establish and maintain effective working relationships with employees, other departments and the public.
  • A strong foundation in evidenced based practice and education and has a responsibility for ensuring adherence to critical care pathways and protocols and ensures an environment of safe quality care.
  • Ability to demonstrate sound clinical judgment, professional communication skills, timely and compliant documentation, and is responsible for professional development and competency validation.
  • Computer skills required.
