Key Responsibilities

1. Care Coordination & Utilization Review

  • Interdisciplinary Team (IDT) Leadership: Collaborate closely with physicians, therapists, social services, and nursing staff to develop, implement, and monitor individualized care plans.

  • Utilization Management: Conduct concurrent clinical reviews with insurance providers (Medicare, Medicaid, Managed Care, and private insurance) to secure authorizations and ensure appropriate utilization of facility resources.

  • Resident Advocacy: Serve as the primary point of contact for families and residents, translating complex clinical goals into understandable steps.

2. Discharge Planning & Transitions of Care

  • Early Transition Planning: Initiate discharge planning upon admission to estimate the length of stay and identify potential barriers to a safe discharge.

  • Community & Resource Liaison: Coordinate post-discharge needs, including ordering Durable Medical Equipment (DME), arranging home health care, scheduling follow-up medical appointments, and coordinating non-emergency medical transportation.

  • Documentation & Compliance: Ensure all discharge paperwork, medication reconciliation forms, and care transitions strictly comply with state and federal regulations.

Position Requirements

  • Education & Licensure:

    • Active, unencumbered state license as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN).

  • Experience:

    • Minimum of 1–2 years of clinical nursing experience in a skilled nursing facility, long-term care, or acute care setting.

    • Prior experience in case management, utilization review, or discharge planning is strongly preferred.

  • Knowledge & Skills:

    • Strong familiarity with Medicare/Medicaid guidelines and Managed Care insurance authorization processes.

    • Excellent communication, conflict resolution, and interpersonal skills.

    • Proficiency with Electronic Health Records (EHR) software (e.g., PointClickCare or MatrixCare).

Physical & Mental Demands
