● Assessing clients’ needs and their eligibility for social programs
● Developing and implementing case management treatment plans
● Maintaining accurate and timely documentation of all services provided
● Answer incoming calls of referrals – documenting and assisting them with placement and following through with their placement to create collaboration with other care partners
● Monitor progress and adjust plans as necessary to ensure effective recovery
● Administrative tasks necessary to upkeep operations in the office
● Act as liaison with outside providers to connect clients with additional resources as needed
● Attend staff/house meetings as requested
● Assist clients with making necessary appointments and maintain daily communication with staff for updates and/or changes
● Work with clients on obtaining their individual goals
● Assist with transition in/out of recovery housing
● Transporting clients as necessary
● Coordinate care with healthcare providers and support services.
Monitor progress and adjust plans as necessary to ensure effective recovery
Facilitating a positive discharge through assisting in securing ongoing care, housing, and other services
Requirements
Proven experience in case management, preferably in emergency medicine or addiction counseling.
Strong understanding of diagnostic evaluation processes and utilization management principles.
Excellent communication skills to interact effectively with patients, families, and healthcare professionals.
Ability to work independently and as part of a team in a fast-paced healthcare environment.
Knowledge of healthcare regulations and compliance standards.
