Job description

Essential Functions: The Program Director is responsible for the day-to-day operations of the Center which would include planning, implementing, supervising, and evaluating the Center programs. The Program Director is also responsible for all staffing in the following areas: direct care, ongoing staff training, clinical (nursing), social services, and housekeeping. The Program Director works closely with the Center team to ensure participant needs are met through Person Centered Care Programming. The Program Director will be responsible for the recruitment, training, and ongoing performance evaluation of all care staff. The Program Director, work with the Social Services Coordinator to tour, assess and enroll participants in the center program and assist families with the decision making involved in utilizing community adult day services. The Program Director is responsible for all budgetary, financial, regulatory and licensure compliance from the various bodies which govern Adult Day Health Care in the State. This position reports directly to the owners. Every effort has been made to make this job description as complete as possible. This employee must maintain effective communication with all staff, participants, caregivers and/or families. Good organizational and time management skills are essential. The employee must maintain a professional attitude and adherence to state licensing policies and procedures. Must have ability to lift, transfer, up to 50 pounds. Must maintain participant confidentiality at all times. Desired Qualifications: Equivalent combinations of education and experience that provide the required knowledge, skills, and abilities will be evaluated on an individual basis. Hired or designated after January 1, 2019, shall meet one of the following qualifications: Bachelors Degree from an accredited college or university and a minimum of 2 years of social services or health services experience and shall have demonstrated ability to perform all aspects of the position: orA licensure by the state of Colorado as a Licensed Practical Nurse or Registered Nurse and completion of two years of paid or volunteer experience in planning or delivering health or social services including experience in supervision and administration; orA high school diploma or GED equivalent, a minimum of four years of experience in social services or health services setting, skills to work with aging adults or adults with functional impairment, and skills to supervise ADS Center staff persons. Preferred Experience: Must have effective written and verbal communication skills.Must have experience in Electronic Medical Records (EMR) platform.Must understand State Regulations on Adult Day Health Care Services. Physical Demands: Ability to sit, stand, walk, bend and squat for prolonged periods; Ability to push and pull objects and lift and carry up to 20 pounds, unassisted, on a frequent basis (occasionally up to 50 pounds); full use of gross and fine motor skills of the upper and lower extremities (i.e., writing in charts, assisting with toileting and incontinence care, assisting with meals, activities, etc.) Working Conditions: Primarily inside in well-lighted, well-ventilated areas Principal Contacts: Center staff, participants, families, and vendors General Requirements: Must be able to execute the terms and conditions set forth in the Center Employee Handbook, including, but not limited to: 1. Works in a safety-conscious manner which ensures that safe work practices are used in order not to pose a risk to self or others in the workplace. Reports any safety violations or hazards to supervisor. 2. Complies with company policies and procedures and local, state, and federal regulations. 3. Adheres to policy on Drug-Free Workplace. 4. Interacts in a tactful, diplomatic, and humanistic manner with participants, families, visitors, and co-workers to provide a safe, efficient, and therapeutically effective caring environment which ensures the self-respect, personal dignity, rights and physical safety of each participant and facility guest. 5. Maintains a dependable attendance record and adheres to standards of cleanliness, grooming, hygiene, and dress code. 6.
