Synergy HomeCare -
Care Relations Coordinator
SYNERGY HomeCare — Middle Tennessee | Murfreesboro & Franklin
Full-time | Office-based | $40,000 base — $45,000–$55,000 total compensation
The Role
Most families come to us at one of the hardest moments of their lives. A parent fell. A diagnosis arrived. The juggling act finally collapsed. They don't just need a caregiver — they need someone who listens, who follows through, and who makes them feel like they're not alone.
That Person Is You
As our Care Relations Coordinator, you are the steady, trusted presence that families and care professionals rely on — the guide in their story. You're not in the background. You're the relationship. You're the one who answers the call, shows up for the consultation, introduces the caregiver, and checks in when no one else does.
What Makes Us Different
You deserve to work somewhere that takes care of you the way you take care of others.
We believe no one should have to choose between caring for others and feeling supported themselves — not our clients, and not our team.
You'll have owners who are genuinely hands-on, a team that values your voice, and the satisfaction of knowing the work you do is making a measurable difference for real families in Rutherford and Williamson Counties and the surrounding areas of Middle Tennessee.
Over 600 families have trusted us since we opened. Most stay until care is no longer needed — which tells you more about our team than any award does. And we've earned a few of those too: five consecutive years as a Provider of Choice, placing us in the top 15% of thousands of agencies nationally based on real client feedback.
What You'll Do
Your days will be varied, relational, and meaningful. Here's how your work comes to life:
Answer incoming inquiries from families and find the right path forward for each one
Conduct care consultations and build initial plans of care for new clients
Introduce care professionals to clients — bridging the relationship from day one
Interview and help onboard new care professionals, setting them up to succeed
Maintain consistent communication with clients, families, and care professionals
Build relationships with local healthcare organizations and referral partners
Partner closely with our scheduling team to ensure seamless care delivery
Review care notes and flag anything that deserves attention
Occasionally provide direct backup care in the field when needed
What You Bring
Hands-on experience with activities of daily living (ADL) or personal care — you know what caregivers and clients actually experience
Strong relationship and communication skills — people trust you quickly, and you follow through
Office and customer service background; comfortable with technology and coordination
CNA and/or Medical Assistant certification preferred (not required)
High school diploma required; some college preferred
Ability to commute to and work from both our Murfreesboro and Franklin offices
Schedule
This is a full-time, 40-hour-per-week position, generally Monday through Friday, 8:00am–5:00pm, with paid overtime available.
Weekend commitment: Two weekends per month, you'll serve as the on-point contact for our care team. In practice, this means you can go about your weekend normally — you just need to be reachable by phone or app, and occasionally by laptop (provided), should an escalation or care consultation need your attention. On rare occasions, a situation may call for a brief in-person visit to a client's home — but this is the exception, not the norm. Weekend on-point availability is compensated separately, in addition to your base salary.

Total compensation (salary + benefits + bonuses): $45,000–$55,000
