Outreach Worker – RWJBarnabas Health

A typical day for an Outreach Worker may include:

  • Meeting patients in a clinic, hospital, ER, and where patients are located and conducting needs assessment using Social Determinants of Health Tool.
  • Scheduling weekly follow-up calls and home visits to patients.
  • Helping patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members lives.
  • Assisting patients with organizing their records, making follow-up appointments with Primary Care Physicians or Specialists.
  • Helping patients with the tasks of getting medical care, and working on health goals by helping to plan healthy meals or finding time to exercise.
  • Helping patients access resources, including quality of care, childcare services, food, housing, transportation.
  • Maintaining relationships with Community Based Organizations.
  • Traveling and meeting members where they are at, which may include local Churches, homeless shelters, and members' residences.
  • Pursuing continued professional development through training, education, and organizational resources that support long-term career growth.

To be considered for this Outreach Community Health Worker opportunity

  • You are passionate about supporting individuals in the community while helping them overcome barriers to meet their health goals.
  • CHW will have a bachelors or equivalent level of education preferred, training in a related social service or human service field, and experience working in the community and with patients with co-morbid conditions
  • Formal Community Health Worker Certification a definite plus
  • Candidate must have the ability to demonstrate the delivery of culturally sensitive services to reduce stigma, ability to provide trauma-informed care and be able to function effectively within a team-based, inter-professional approach,
  • A valid drivers license is required for this position and employees will be required to conduct outreach and support services outside of traditional work hours, such as evenings/weekends
