Pay Rate: $ 2400 To $ 2732 per week

About the Role
We are seeking an experienced Physical Therapist (PT) for a Home Health contract position serving patients throughout Solano County, California. The PT will provide home-based physical therapy services, prioritize start-of-care visits, supervise Physical Therapy Assistants, and maintain timely clinical documentation and communication.
This position requires an active California Physical Therapist license and at least one year of Home Health experience. The role has a productivity expectation of 30 points per week, with caseloads and visit mixes structured around an average 40-hour workweek.
Responsibilities
Prioritize and complete start-of-care visits according to clinical and scheduling requirements.
Supervise Physical Therapy Assistants providing patient care within the assigned territory.
Complete start-of-care reporting by the following day and other clinical documentation within 48 hours of each visit.
Maintain timely communication with supervisors, intake staff, and office personnel during normal business hours.
Manage assigned Home Health visits and productivity requirements while covering Solano County and expanding the territory as needed.
Requirements
Active California Physical Therapist License Required.
Minimum 1 Year of Home Health Experience Required.
Ability to work a 40-hour Saturday Wednesday schedule.
Ability to supervise Physical Therapy Assistants as required.
Ability to meet required clinical documentation, reporting, communication, and productivity standards.
The successful Physical Therapist will provide timely, well-documented Home Health therapy services while supporting continuity of care across the assigned territory
